FORMS (Field Operations and Records Management System) II Lite is developed
and maintained by the US Environmental Protection Agency (EPA), Analytical
Operations/Data Quality Center (AOC), Office of Solid Waste and Emergency
Response (OSWER).
The FORMS II Lite software system is an EPA AOC developed, stand-alone software
program to support the mission of EPA and its contractors who visit hazardous
waste sites to take samples of soil, water, air, and other sample matrices.
FORMS II Lite version 5.0 currently creates output files that hold all of
the data contained in the Chain of Custody (COC) forms. FORMS II Lite's
electronic chain of custody data is provided as an eXtensible Markup Language
(XML) export for laboratory and Regional COC form copies. This process assures
the same site confidentiality as hardcopy COC forms.
The goal of the F2LIMS software is to reduce the level of effort used in
entering data from a hardcopy COC into the LIMS by capitalizing on the electronic
data provided in the FORMS II Lite COC XML file.
F2LIMS is a software application that allows the user to:
- Import the FORMS II Lite XML file;
- Translate analyses and matrices based on user set definitions automatically;
- Incorporate edits to the sample data easily;
- Export the data in a variety of custom LIMS-ready formats; and,
- Preview the electronic COC data in a standard format.
Minimum System Requirements
- IBM-compatible;
- Windows 98, NT, 2000, or ME;
- Internet Explorer 4.0 (or higher);
- 200 MHz Pentium processor;
- 32MB RAM (64 MB recommended);
- 35MB available hard disk space;
- 2X speed (or faster) CD-ROM drive;
- 800x600 resolution using small fonts; and,
- Mouse.
In order to protect data integrity and reduce laboratory data entry, a FORMS
II Lite XML file may be provided to the laboratory with the samples. Laboratories
are then able to import, enter, and modify chain of custody data directly
into F2LIMS software. The data can then be exported in a LIMS-ready format
to be imported by the LIMS and replace the Sample Login process.
FORMS II Lite can be used to document samples shipped to Regional, Contract
Laboratory Program (CLP), or commercial laboratories for all Superfund analytical
services or generic sampling activities. Since October 1, 2000, nine of
the ten EPA Regions have used FORMS II Lite to document sampling activities
and generate paperwork for over 25,000 samples shipped to CLP laboratories.
During FY02 almost half of the samples shipped through the CLP were documented
using FORMS II Lite. In addition, several Regions have used FORMS II Lite
to generate paperwork for samples shipped to their Regional and/or commercial
laboratories.
AOC intends to eliminate the use of the current hardcopy Traffic Reports
and make the use of FORMS II Lite mandatory for generating all CLP paperwork.
AOC is also evaluating the feasibility of making FORMS II Lite the sole
mechanism for creating sample paperwork for all samples collected from Superfund
sites. The target date for implementing these changes is October 1, 2002.
